CityLife Health connects Medicaid members to what matters most to them. By providing quality walk-in care and operating extended hours, the clinic eliminates the need to wait hours in the emergency department for minor healthcare needs or basic primary care. More importantly, CityLife’s community health workers connect members to valuable resources and services to help improve their lives. Focused on the core tenants of health, shelter, clothing, and food, CHW’s do everything from assisting with medical benefits to initiating contacts for safe housing to outfitting members with clothing for an interview through “Eli’s closet” to sharing a fresh, healthy food while awaiting SNAP benefits.
CityLife works with community partners to provide aggregation points to provide access to resources and for point of care testing and other minor medical needs. Through education and community involvement, CityLife hopes to help Medicaid members improve and be proactive about their health
Conveniently located at the corner of North Olden Avenue and New York Avenue, the clinic is easily accessible by bus.
